Client Care Coordinator
Own the day-to-day client experience for South West Georgia Homecare. You will run intakes, match caregivers to families, build the schedule, and be the named person families can reach directly.

What you will do
- Complete client intakes and in-home service assessments
- Match caregivers to clients by skills, personality, and proximity
- Build and maintain the weekly caregiver schedule
- Serve as the named point of contact for families
- Coordinate hospital discharge and referral requests
- Document service changes and escalate condition changes same day
What you need
- Two or more years of scheduling, care coordination, or healthcare operations experience
- Excellent phone and written communication with families
- Comfort working in scheduling and electronic record systems
- Strong organization under shifting daily priorities
- Knowledge of Dougherty, Lee, and surrounding counties preferred
- Ability to pass background screening
